We are seeking a full time RN for our MDS – Clinical Services Coordinator position in our, non-profit senior living community, located in Bridgeton, MO.

Summary of position: Coordinates the completion of the MDS process with the interdisciplinary team and monitoring of the facility QI indicators.  Coordinates and schedules careplan conferences with staff, residents and responsible parties in coordination with social services.  Develops and implements resident careplans and ensures that the careplan interventions are followed by the appropriate disciplines.  Serves as the chairperson for the falls program/committee.  Monitors and intervenes with weight loss and the completion of monthly weights on all residents in the facility.  Assures the compliance of the facility with the infection control procedures involving Heptovax, PPD, Flu Vaccine and immunizations.  Coordinates with the Director of Nursing to ensure the quality of nursing care is provided to each and every resident in Anna House and Elizabeth House in accordance with current federal, state and local regulations and facility policies.

Job Requirements

Essential Functions:

  1. Understands the importance of customer service and possesses the willingness to learn resident/relative satisfaction techniques.

  2. Performs basic nursing skills with knowledge and competency.

  3. Listens to residents, their families and physician concerns and initiates corrective action and /or reports concern to Director of Nursing.

  4. Conducts inspections of all nursing service functions to assure that quality control measures are continually maintained.

  5. Identifies safety hazards and initiates corrective action immediately.

  6. Participates in survey inspections made by authorized government agencies and aides in implementing a plan of correction for deficiencies noted during these inspections.

  7. Ensures that the nursing services Policy and Procedure manual is followed by the nursing personnel.

  8. Interprets existing policies and procedures to charge nurses, nursing assistants, restorative aides, residents, families and physicians.

  9. Attends staff meetings, in-service classes and committee meetings as assigned.

  10. Directs the implementation and observation of facility infection control procedures.  Maintains staff records for Heptovax, PPD's.  Monitors resident records for PPD and Flu Vaccine.  Coordinate the administration of the Flu vaccine to employees and residents yearly.

  11. Directs implementation and observation of facility safety policies and procedures.

  12. Directs implementation and observation of facility disaster preparedness procedures.

  13. Directs implementation and observation of resident rights policies, as well as maintaining resident confidentiality.

  14. Directs, evaluates and supervises resident care and initiates corrective action as necessary.
